Patient Information

Our staff looks forward to meeting you and discussing your goals. We strive to keep our appointments on time as scheduled. Prior to your appointment, we encourage you to fill out the patient forms that are located on this website so you can accelerate your check-in process. Please download, print, and complete the forms and bring them with you to your appointment. If you are unable to download our patient forms, then it is recommended that you arrive at your appointment 15 – 30 minutes early so you can have ample time to fill out the appropriate forms. For parking, access is in either parking lot or garage on Paul St.

Your Initial Visit
As a new patient, we will evaluate your present oral health and listen to your dental concerns. You will be asked to complete a medical/dental, periodontal and oral examination, the necessary radiographs will be taken in order to properly identify and diagnose any existing or developing problems. If needed, we will then provide you with all of your treatment options. Together we can decide upon the best possible treatment plan for you.


Appointments
Our goal is to perform your dental services by setting aside the appropriate time for your appointment in advance. We respect your time and make every effort to remain on schedule. If you are unable to keep your appointment or are going to be late, kindly notify our office immediately. Except in the case of an emergency, we require a minimum of one full business day advance notice for cancellation or to change an appointment.


Financial policy
We will make every attempt to quote fees before beginning treatment. In the case of one visit preventive, restorative or cosmetic procedures, payment is due at the time services are rendered. If your treatment is more extensive, a payment plan can be arranged with our office manager. For your convenience, we accept most major credit and debit cards.


Insurance
We will be happy to provide assistance and submit all insurance claims for your dental visits. This will enable you to receive the maximum benefit from your insurance company. We will need your assistance in order to better understand your coverage and make this process more efficient. In order to help you understand your co-pay and avoid potential misunderstandings, we recommend that you speak with your benefit office and bring your “dental benefit booklet” insurance form and insurance card so we can review your dental coverage.


Emergency care
We understand that emergency dental problems may arise at anytime. Our office is always covered and we will respond quickly to your needs. In the event of a dental emergency, please phone the office and you will be given our office page number. After calling the number, you will hear a long tone; please dial the phone number where you can be reached followed by the # key. We will respond and return your call as soon as possible.
